These Terms and Conditions apply to services provided by Crystal Clear Diagnostic Solutions, PLLC. By booking an appointment, receiving services, submitting forms, making payment, or communicating with our team, the client agrees to these Terms and Conditions.
Services Provided
Crystal Clear Diagnostic Solutions, PLLC provides specimen collection, diagnostic screening support, wellness lab collection support, DNA collection services, drug testing collection services, workplace testing support, and related testing coordination.
Services may include:
• Wellness lab collection
• Blood draw services
• Pre-ordered lab collection support
• Legal and non-legal DNA collection
• Paternity, maternity, sibling, avuncular, grandparent, and immigration DNA collection
• DOT and non-DOT drug testing collection
• Urine, rapid, hair, nail, and oral fluid drug testing collection
• STD and wellness lab collection support
• Phlebotomy services
• Employer testing support
• Mobile specimen collection, when available
Services are by appointment only at our approved location or approved testing location. Same-day appointments may be available but are not guaranteed.
Appointments and Scheduling
Appointments must be scheduled ahead of time by phone, email, website, scheduling link, or approved booking platform.

A 10-minute grace period may be allowed. Late arrivals may need to reschedule.
No-shows or cancellations with less than 24 hours’ notice may result in a rescheduling fee.
Walk-ins are not guaranteed.
Payment Terms
Payment is due at the time of service unless there is a written agreement.
Accepted payment methods may include credit card, debit card, approved electronic payment, or approved business payment method.
DNA testing, drug testing, wellness lab collection, mobile services, specialty testing, and other collection services may require payment in full before collection.
Payments are final. No refunds are issued once the service is rendered, a sample is collected, a lab order is processed, a testing kit is shipped, or chain-of-custody paperwork has been started.
Legal and Court-Admissible Testing
Crystal Clear Diagnostic Solutions may work with accredited laboratories or authorized testing partners for legal DNA testing so results can meet chain-of-custody and court standards.
The client must tell Crystal Clear Diagnostic Solutions at scheduling if the test is for legal use.
Non-legal tests may not be accepted in court or for legal matters.
Crystal Clear Diagnostic Solutions is not responsible for delays caused by incorrect, incomplete, or missing client information, documents, identification, consent forms, or testing paperwork.
Confidentiality and Privacy
Personal information and test-related information are kept confidential according to applicable privacy standards, industry standards, and company privacy practices.
Results are released only to authorized parties listed on the order, chain-of-custody form, consent form, employer agreement, testing agreement, or legal document.
Crystal Clear Diagnostic Solutions will not release results to a third party without written consent, legal authorization, or other authorization required by law.
Result Delivery Times
Result turnaround times vary based on the test type, lab partner, shipping, holidays, compliance review, medical review officer review, and processing requirements.
Estimated result times may include:
• DNA results: usually 2 to 5 business days after the lab receives the specimen
• Rapid drug screens: may be available the same day or within minutes, depending on the test type
• Hair, nail, oral fluid, and lab-based drug tests: usually 2 to 5 business days after the lab receives the specimen
• STD and wellness lab results: usually 1 to 5 business days, depending on the test type and lab partner
Delays may happen because of lab processing, holidays, shipping, incomplete documents, specimen review, compliance review, medical review officer review, or other issues outside of our control.
Client Responsibilities
Clients agree to:
• Provide accurate and truthful information
• Bring valid identification when required
• Provide required orders, forms, authorizations, or consent documents
• Follow staff and collector instructions
• Respect staff, other clients, and the testing environment
• Not use services for fraud, illegal activity, deception, or tampering
• Notify Crystal Clear Diagnostic Solutions of any concerns that may affect specimen collection
Limitation of Liability
Crystal Clear Diagnostic Solutions is not a diagnostic laboratory unless specifically stated.
Crystal Clear Diagnostic Solutions acts as a specimen collection provider, testing support provider, and coordination service for third-party laboratories, testing platforms, medical review officers, compliance partners, or authorized testing partners.
Crystal Clear Diagnostic Solutions is not responsible for lab delays, inconclusive results, rejected specimens, invalid results, third-party processing issues, legal decisions, employment outcomes, medical decisions, court decisions, immigration decisions, or other consequences connected to test results.
Refusal of Service
Crystal Clear Diagnostic Solutions reserves the right to refuse or discontinue service to anyone who is:
• Disrespectful
• Threatening
• Disruptive
• Under the influence of drugs or alcohol
• Refusing to follow safety rules
• Refusing to follow collection instructions
• Unable or unwilling to provide required identification
• Unable or unwilling to provide required forms or consent
• Requesting something outside our scope of service
• Suspected of fraud, tampering, or unsafe behavior
Age Restrictions
Clients must be 18 or older to authorize testing unless otherwise allowed by law.
For minors, a parent, legal guardian, court representative, or other authorized adult must give consent and provide proper documentation when required.
Crystal Clear Diagnostic Solutions may deny or delay service if proper consent, identification, or documentation is not provided.
Re-Testing and Disputes
Retesting may require an additional fee.
If a specimen is rejected because of client error, insufficient sample, refusal to follow instructions, tampering, incomplete paperwork, or missing documentation, the client is responsible for recollection fees.
Additional fees may apply for recollection, retesting, lab processing, mobile service, shipping, administrative processing, or third-party partner fees.
Result disputes must be directed to the testing laboratory, medical review officer, employer representative, compliance partner, or authorized testing partner when applicable.
Crystal Clear Diagnostic Solutions does not guarantee that a recollection or retest will produce a different result.
Governing Law
These Terms and Conditions are governed by the laws of the State of Illinois.
Disputes are handled according to applicable state and federal laws.
Changes to Terms
Crystal Clear Diagnostic Solutions reserves the right to update these Terms and Conditions at any time.
The newest version will be posted on our website.
Continued use of our website, booking system, forms, payment links, or services means the client accepts the updated Terms and Conditions.
