Testing, DNA Collection, Drug Screening, and Retesting Policy
Crystal Clear Diagnostic Solutions provides professional specimen collection and testing support services. Depending on the service requested, testing may be completed through third-party laboratories, testing partners, medical review officers, or authorized compliance partners.
All clients are responsible for providing accurate information, valid identification when required, and any completed forms, orders, lab orders, or authorizations needed for testing.
Appointments
Services are provided by appointment only unless otherwise stated.
Clients are responsible for arriving on time and bringing any required identification, lab order, consent form, employer authorization, or testing documentation.
Late arrivals may need to be rescheduled. Missed appointments or late cancellations may result in forfeited payment or a new service fee.
Wellness Lab Collection
Wellness lab collection services may be completed through third-party laboratories or testing platforms.
Some tests may require a provider order or authorization through a third-party lab platform.
Clients are responsible for following any preparation instructions provided before testing. This may include fasting, timing requirements, hydration instructions, or other lab-specific instructions.
Crystal Clear Diagnostic Solutions is not responsible for delayed, rejected, or incomplete testing caused by missing orders, incorrect client information, missing forms, or failure to follow preparation instructions.
DNA Testing and DNA Collection
For legal DNA testing, all participants must provide valid government-issued photo identification at the time of collection.
Legal DNA collections may require chain-of-custody documentation. Once collection has started, the service is considered initiated and is non-refundable.
DNA results are released only to the authorized party listed on the test order, consent form, legal documentation, or payment record. Results will not be released to family members, partners, friends, or other third parties without proper written authorization or legal documentation.
Crystal Clear Diagnostic Solutions does not provide legal advice, custody advice, immigration advice, or interpretation of DNA results.
Drug Testing and Workplace Testing
Drug testing services may include rapid screening, urine testing, hair testing, nail testing, oral fluid testing, or other collection methods depending on the service requested and laboratory requirements.
For employer-based or workplace testing, results may be released directly to the designated employer representative, human resources contact, medical review officer, compliance partner, or other authorized party listed on the order.
Crystal Clear Diagnostic Solutions does not release employer-based drug testing results directly to employees or donors unless authorized by the employer, testing agreement, medical review officer, or applicable law.
All questions about workplace testing results, return-to-duty requirements, follow-up testing, compliance requirements, or employment decisions must be directed to the employer, medical review officer, substance abuse professional, or compliance partner.
Refunds and Cancellations
All testing and collection services are non-refundable once:
• A specimen has been collected
• A test has been started
• A laboratory order has been processed
• A testing kit has been shipped
• Chain-of-custody paperwork has been started or completed
• A third-party lab or testing partner has accepted the order
• A mobile appointment has been dispatched
Cancellations or rescheduling requests should be made at least 24 hours before the appointment time.
Missed appointments or late cancellations may result in forfeited payment. A new appointment may require a new service fee.
Shipping fees, mobile service fees, administrative fees, and third-party laboratory fees are non-refundable.
Result Turnaround Times
Result turnaround times are estimates only and are not guaranteed.
Results may be delayed due to laboratory processing, specimen review, holidays, shipping, courier delays, compliance review, medical review officer review, employer requirements, legal requirements, or other factors outside of our control.
Crystal Clear Diagnostic Solutions does not control third-party laboratory processing times.
Inconclusive, Invalid, Rejected, or Insufficient Results
An inconclusive, invalid, rejected, or insufficient result may occur for reasons outside of the collection site’s control. This may include insufficient specimen volume, specimen integrity issues, laboratory processing limitations, donor-related factors, shipping issues, or other issues identified by the laboratory or testing partner.
If a result is inconclusive, invalid, rejected, or insufficient, recollection or retesting may be required.

Crystal Clear Diagnostic Solutions does not guarantee that a recollection or retest will produce a different result.
Mobile Services
Mobile services may be available by appointment only.
Mobile appointments may require advance payment, travel fees, mileage fees, parking fees, toll fees, or after-hours fees.
Mobile service fees are separate from lab testing or collection fees unless otherwise stated.
Mobile appointments may be delayed, canceled, or rescheduled due to weather, traffic, safety concerns, staffing, location access issues, or circumstances outside of our control.
Testing Area Policy
For privacy, safety, infection control, and chain-of-custody purposes, only the person being tested may enter the collection area unless a parent, guardian, legal representative, translator, or approved support person is required.
Children, guests, and additional parties may not be allowed in the collection area during drug testing, DNA collection, or other specimen collection services.
